Aeris - AT-400 First Stage Regulators

October 23, 2007– San Leandro, CA – AERIS has been notified by its supplier (ROMI, Ent.) that they have received isolated reports of AT400 type Regulator First Stages exhibiting uncontrolled flow of air to the Second Stage.

Evaluation has shown that a specific batch of HP (High Pressure) Seats contained material that can deteriorate and fail to properly perform its mechanical sealing (shut off) function, causing an uncontrollable free flow which could result in serious injury to or death of the user.

In cooperation with the U. S. CPSC (Consumer Product Safety Commission), ROMI is initiating a voluntary recall of all affected AT400 First Stages.

The affected First Stages include those that were shipped to AERIS Retailers from the factory between May 1, 2006 and November 15, 2006. Other Diaphragm type First Stages that received annual service between May 1, 2006, and October 22, 2007, may also be affected.

NOTE: Due to the random nature utilized to assign SNs (serial numbers) to First Stages, there is only a small quantity of affected units within the large range of SNs. Since the extent of listing the actual SNs of all affected units would be impractical and not user friendly, AERIS has created a data base on the diveaeris.com web site that product owners and retailers can easily access to determine whether a specific First Stage is affected and if it has received the retrofit.

If you have an affected unit, you must discontinue further use of it and take it to the AERIS Dealer where you purchased it, or a local Authorized AERIS Service Center, for a component retrofit at no charge.

If you no longer own the Diaphragm type Regulator First Stage, please forward this letter to the new owner or have them contact us directly.

WARNING: DO NOT CONTINUE USE of the Regulator First Stage until it receives a Retrofit. Only Authorized AERIS Dealers and their trained service technicians must perform this Retrofit. Non authorized persons who attempt the Retrofit procedure risk voiding the product’s warranty and rendering the product dangerously unsafe for SCUBA use.

Oceanic - FDX-10 and CDX-5 First Stage Regulators

October 23, 2007– San Leandro, CA – Oceanic has been notified by its supplier (ROMI, Ent.) that they have received isolated reports of DX type Regulator First Stages exhibiting uncontrolled flow of air to the Second Stage.

Evaluation has shown that a specific batch of HP (High Pressure) Seats contained material that can deteriorate and fail to properly perform its mechanical sealing (shut off) function, causing an uncontrollable free flow which could result in serious injury to or death of the user.

In cooperation with the U. S. CPSC (Consumer Product Safety Commission), ROMI is initiating a voluntary recall of all affected CDX5 and FDX10 (diaphragm) type First Stages.

The affected First Stages include those that were shipped to Oceanic Retailers from the factory between May 1, 2006 and November 15, 2006. Other model DX type First Stages, such as the CDX, DXi, DX3, DX4, and TDX5, that received annual service between May 1, 2006, and October 22, 2007, may also be affected.

NOTE: Due to the random nature utilized to assign SNs (serial numbers) to First Stages, there is only a small quantity of affected units within the large range of SNs. Since the extent of listing the actual SNs of all affected units would be impractical and not user friendly, Oceanic has created a data base on the OceanicWorldwide web site that product owners and retailers can easily access to determine whether a specific First Stage is affected and if it has received the retrofit.

If you have an affected unit, you must discontinue further use of it and take it to the Oceanic Dealer where you purchased it, or a local Authorized Oceanic Service Center, for a component retrofit at no charge.

If you no longer own the DX (diaphragm) type Regulator First Stage, please forward this letter to the new owner or have them contact us directly.

WARNING: DO NOT CONTINUE USE of the Regulator First Stage until it receives a Retrofit. Only Authorized Oceanic Dealers and their trained service technicians must perform this Retrofit. Non authorized persons who attempt the Retrofit procedure risk voiding the product’s warranty and rendering the product dangerously unsafe for SCUBA use.

OMS - BCD Overpressure Valves

From OMS:

Important Product Safety Recall : Buoyancy Control System Overpressure Valve (OPV)

In cooperation with the U.S. Consumer Product Safety Commission:

OMS BC’s with the following serial numbers { located on the WARNING LABEL } :

OM1200000 - OM1200085
OM0101000 - OM0101229
OM0201000 - OM0201269
OM0301000 - OM0301159
OM0401085 - OM0401319

OM0601125 - OM0601351

Molding variations in one component of the overpressure valve (OPV) used in the OMS Buoyancy Control Systems (BCS) could, in rare instances, prevent the OPV from sealing properly creating a potential drowning hazard. While this variation is not present in every OPV, we feel it may be difficult to determine in the field which OPVs are suspect. Please return the BCS overpressure valve to an authorized dealer for a free replacement OPV.

Suspect overpressure valves may have a RED pull.

New replacement OPV will have a BLACK pull.

Promate Regulator Swivel

May 31, 2007

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Swivel for a Scuba Regulator

Hazard: The swivel, which is attached to a diving regulator, could separate while diving. This will result in a complete
and sudden loss of the diver’s air supply, causing the diver to engage in emergency ascent procedures. This poses a risk
of decompression sickness due to rapid ascent, and air embolism or drowning if the diver panics or the emergency ascent
procedure fails.

Incidents/Injuries: The firm has received one report where the swivel separated during a dive and caused the diver to ascend using the buddy breathing technique. No injury was reported.

Description: The recall involves the HO110 Swivels sold as an aftermarket regulator component. There are no markings
such as a date or production code embossed on the unit.

Sold at: Specialty retail dive stores nationwide from January 2006 through March 2007 for about $40. They also could have been installed by a dive shop regulator technician.
Manufactured in: Taiwan

Remedy: Consumers should stop using regulators with the swivel attached and contact the dive store where purchased for a refund.

Consumers can easily identify if the service upgrade has been completed by counting the vents on the plastic saddle located on the regulator. If the service upgrade has not been completed, the consumer should bring the regulator to any authorized Scubapro dealer to have the service upgrade completed free of charge.
Consumer Contact: For assistance in locating the nearest authorized Scubapro dealer or for more information, contact Scubapro USA toll-free at (800) 731-6685 between 8 a.m. and 5 p.m. PT Monday through Friday, or go to the firm’s Web site at www.scubapro-uwatec.com.

September 7, 2005 Ocean Technology Systems - M & J has issued a Recall notice for all Swivels manufactured during the time period of January 2004 through February 2005. This notice is for the M & J after market swivel designed to be used with the Interspiro Divator MKII full face mask and a standard diving type supply hose.

DO NOT DIVE ANY M & J SW-3000 (SW-1 stock number SR-109-AGA) swivel until further directed by M & J Engineering and especially if you have noticed the swivel unscrewing during normal use in or out of the water! In a recent meeting with M & J Engineering Staff on August 15, 2005 (hereinafter known as M & J), it was determined that the swivels have a manufacturing tolerance problem that needs to be corrected. The manufacturer, M &  J suggests that you connect the swivel to a Interspiro 2nd stage regulator (which is what the said swivel was designed to operate with) and hand tighten the swivel connector to the 2nd stage regulator. After tightening and turning on your air supply, you should feel a .010 play as you try and move the hose in &  out from the swivel. If you experience no play at all, your swivel needs to be replaced. DO NOT USE THE SWIVEL IF YOU ARE NOT 100% SURE IT ISSAFE!

Who to Contact for Warranty Service, Replacement and/or Questions: M & J Engineering, 12520 Kirkham Court (Ste #1), Poway, California 92064, Telephone number 858-748-9442, Fax 858-679-1632, Email: sales@omniswivel.com. Sales Manager: Mr. Joseph De Wit.

If you have any comments or questions, please email and/or call us. Please note OTS is not going to handle the recall for this after market product.

July12, 2005 Kirby Morgan Dive Systems, Inc. (KMDSI) has learned that a single batch of non-return (oneway) valves manufactured by Circle Seal Controls, Inc. may be defective. These valves are used on all Kirby Morgan masks and helmets manufactured for surface-supplied diving. The valves in question are date stamped on the valve body and fall within the date range 12/04 to 02/05. If you have a valve like this on your helmet, YOU SHOULD STOP USING IT IMMEDIATELY and replace it with a valve that does not fall within this date range.

The purpose of these valves is to prevent a back-flow of breathing gas up the diver’s hose in the event that the hose is severed between the diver and the surface. If a diver’s hose is severed, a properly functioning non-return valve will automatically close and prevent the air within the helmet from being lost.

The valve also prevents the diver’s emergency gas supply (bail-out) from venting out through the non-return valve.

In the defective valves, small slivers of the Teflon wiper (used in these valves as part of the sealing mechanism) may be cut off the Teflon wiper and interfere with the poppet that seals the valve. When this occurs it is possible for air from the helmet or emergency gas supply to be lost through the valve if the diver’s umbilical has been severed or umbilical pressure is lost for any reason. This could lead to the diver suffering severe personal injury, known as a squeeze, or death.

Kirby Morgan has used the Circle Seal non-return valve in its commercial diving masks and helmets for over 30 years. Kirby Morgan tests every non-return valve on completed units prior to shipment, but the damage to the wiper may occur when the valve has been overhauled, disassembled and re-assembled.  Since this valve is also an industry standard, you may find it on other manufacturer’s helmets as well. If this is the case, you should contact that manufacturer for a replacement.

If you have one of these valves on your helmet, or are carrying a valve like this as a spare, DO NOT DIVE WITH IT! Please return it immediately to KMDSI, or any of its dealers, for a free replacement.

KMDSI can be reached at 1430 Jason Way, Santa Maria, CA 93455. Telephone 805-928-7772, FAX 805-928-0342. Email kirwin@kirbymorgan.com. Website www.kirbymorgan.com.

May 25 2004 - Halcyon Stainless Steel Power Inflator Recall

http://www.halcyon.net/recall/index.shtml

Halcyon is recalling all stainless steel power inflators. The only units affected can be easily identified by their stainless buttons and started shipping on all Halcyon BCs after Oct 21, 2003. This recall is in response to problems experienced by some Halcyon customers. Identification of the cause was unusually difficult due to the odd dispersion of these problem units. Furthermore most customers report that their units are performing flawlessly. However, there are pockets of unacceptable failures in which customers or dive shops seem to have a grouping of unreliable inflators. Given the nature of this failure, there is no way to ensure that a unit performing properly will not develop problems over time.  Typically, the problem expresses itself as a slow leak into the BC bladder. The cause of the leak relates to small imperfections within the machining of the stainless air barrel of some inflators. Not all of the inflators exhibit the machining flaws. Thus, a majority of the units perform flawlessly but groupings of product might display the imperfections. We believe there to be no responsible alternative but to institute a recall of every stainless steel inflator sold by Halcyon. In order to ensure this recall occurs in the most responsible manner possible, we have contacted the U.S. Products Safety Commission and notified them of the return. They are monitoring the recall process and the return of the units.Halcyon is deeply apologetic for any inconvenience this recall might cause. We are dedicated to making the world's best diving products and to ensuring diver safety at all costs; this solution seems the only reasonable course of action. Please do not use your BC with the Halcyon stainless steel power inflator until we have had the chance to replace the suspect unit. This recall only applies to Halcyon inflators shipped after Oct 21, 2003. These units have stainless steel buttons and are easily identifiable from previous Halcyon inflators.

In order to reduce the inconvenience and to return your BC to operation as quickly as possible, we are shipping new inflators to your local Halcyon dealer. You may bring your inflator to your dealer, trade it for a new unit, and have it fitted to your buoyancy compensator. Please contact your local Halcyon facility to coordinate the best time for this warranty replacement.

If this solution is not feasible, please contact Halcyon using the following information:

386-454-0811   or    800-Halcyon (425-2966)  or   techservices@halcyon.net

If you prefer to send your Halcyon inflator directly to Halcyon please call or email to obtain a return authorization number. This number will significantly assist in the speedy return of your new Halcyon inflator.

Please include your return authorization number with your return and send the unit to the following address:  Halcyon Manufacturing

April 8, 2004 - Aqua Lung ABS Octopus Exhaust Grid Replacement -  http://www.aqualung.com/technical_library/ABSRetrofitInstoreP oster.pdf

Aqua Lung has determined that it is possible, under certain circumstances, for one or both of the exhaust valves on the ABS Octopus to become inverted. Inverted means that the edge of the exhaust valve can get lodged between two of the spokes of the seating surface. This could happen during the after-dive rinsing process if the valves are hit with a stream of high pressure water from a garden hose or it the valves hit the water just right during an entry. The result of having one or both valves inverted is the inability to completely seal out water. This could lead to an aspiration of water, which in turn could lead to panic, injury or death. The valves will not invert during the course of a dive.

To protect the valve from inverting, Aqua Lung has redesigned the exhaust grid that covers the exhaust valves (see picture on website)

Aqua Lung is requesting that all ABS Octopus regulators be brought into your local authorized Aqua Lung Dealer. The dealer will change out the exhaust grid at no charge. The replacement procedure is quick and in most cases can be done while you wait. The procedure is non-invasive and will not alter the performance or the adjustments of the regulator in any way.

Aqua Lung apologizes for any inconvenience that this may cause you. Your safety is our top priority.

Aqua Lung America, 2340 Cousteau Court, Vista, CA 92081 (760) 597-5000

April 2004 -  Sea & Sea DX-3100 Camera - We have been informed by our Research and Development department of the possibility of cracks appearing in the battery compartment door or/and in the USB compartment door of the DX-3100 Aquapix amphibious camera that may lead to water leakage. Although we have only received a limited number of these reports, even the remotest possibility of this condition appearing has prompted Sea&Sea to take affirmative and positive action and therefore we would like to request that you return your DX-3100 Aquapix to the Sea & Sea dealer where it was purchased, or to an official Sea&Sea distributor for testing and any necessary upgrading if within these corresponding serial numbers. Corresponding Serial Numbers: 030200001-030204016   Although all our products are pressure tested before leaving the factory, our Research Department determined that the reported cases resulted from

micro cracks, caused by physical stress after a certain period of time, that formed in the battery compartment door.

Sea&Sea would like to apologize for any inconvenience this may cause you, and will make every effort to ship back your camera as soon as it could in cooperation with Sea&Sea official distributor in your region.

March 16, 2004 - Powerline Airway Ribbed Hose Replacement

http://www.aqualung.com/technical_library/PowerlineRetrofit.pdf

Aqua Lung has received reports that some of the ribbed hoses (Fig. 1) on the Powerline Airway have been tearing at the top near the dual exhaust valve. The problem has been traced to an error in the molding process from our supplier that occurred over a three-month period between 2001 and 2002. The best way to confirm whether or not you have one of the questionable hoses is to look at the serial number on the lower unit of the Powerline (fig. 2). If the serial number falls within the range of M48H to M52H (M48H, M49H...M52H), or M01I to M08I (M01I, M02I...M08I) and is the original hose, then it should get replaced. This affects both SeaQuest and Aqua Lung BCs utilizing the Powerline inflator.

If the ribbed hose were to tear near the top, the BC would not be able to hold air and a loss of buoyancy would occur. This could result in the need for the diver to ditch their weight pockets or rely on their buddy for assistance. In extreme cases it could result in result in panic, injury or death.

If you have a spiral hose that falls within the serial number range, please have the hose replaced by this authorized Aqua Lung retailer.

January 2004 -- Sherwood or Genesis BCDs Recalled  - If you purchased a Sherwood or Genesis BCD after July 1, you might have a defective inflator valve that could continuously inflate, a serious problem.

The affected models have a black or gray inflator button and are on Sherwood Silhouette, Magnum, Avid Luna, and Freedom models, with serial numbers 90010001 to 90350167, and the Genesis Cayman, Cobra, Athena, and Phantom (same serial numbers). To find the serial number, check the "Important Notice" label, which may be inside or behind the BCD pocket. A spokesman said they discovered the defect during quality control checks at a Thailand manufacturing facility. About 90 percent of the affected units were still in warehouses, and so they were upgraded with a blue colored button before being shipped. If you believe you have a defective BCD,  return it to your dealer or call 800-815-9740, ext. 233.

KMDSI Part # 505-055 (and pre '89 505-003)

Kirby Morgan Dive Systems, Inc. (KMDSI) was alerted by its Authorized Dealer in Southern Africa about the following incident: A Kirby Morgan SuperLite 17A/B (Serial #82929, purchased in April 1997) neck clamp assembly was found jammed in a container door. The neck clamp was hanging on the umbilical rack on the door and the person closing the container door did not notice that it was jammed. When the jammed neck clamp was discovered, it was removed and closely inspected. During the clamping process (onto the helmet) one of the welded pins on the neck clamp assembly broke off. On closer inspection it was noted that there was no way of identifying this potential break as both sides of the pin are welded.

It is important that a visual inspection of the neck clamp assembly be done before every dive. Normal inspection of the neck clamp assembly is covered in the Kirby Morgan Daily Pre-Dive inspection Appendixes A2.3, Monthly Inspection A2.2 and the Annual / 400 Hour Overhaul A2.1. These Appendixes can be found under notices/maintenance checklists.

The Annual/400 hour Overhaul has the technician remove the clamp from the yoke and neck dam so that all the component surfaces are revealed for inspection. The monthly Inspection check has the technician checking all welds and checking the entire clamp adjustment stud by loosening the lock nut and exposing the entire stud. This check is not as complete as the annual but is considered adequate if done monthly. Neck clamps older than five years should be inspected weekly in accordance with Appendix A2.2 steps 1-6 and daily in accordance with Appendix A2.3 Daily Pre-Dive Set up and Inspection steps 1.1-1.2. Anytime neck clamps are used on different helmets the neck clamp must be checked for proper function, fit and adjustment prior to diving.

Over the years, KMDSI has upgraded and enhanced the SuperLite® 17 A/B, MK 21 neck clamp assembly. Enhancements include not only more stringent inspections, but reinforcements in critical areas as well as changing materials to aid in the reduction of oxidizing. Since April, 1999 KMDSI has inscribed every SuperLite® 17 A/B, MK21 neck clamp with date of manufacture and an identification number.

April 2004 will be five years since the inception of inscribed neck clamps. If your neck clamp is not inscribed, it is older than five years and KMDSI strongly recommends that the neck clamp be inspected on a weekly basis in accordance with SuperLite 17 A/B monthly helmet inspection Appendix A2.2 procedures 1 6.

Any Neck Clamp Assembly that does not pass inspection and is not repairable should be destroyed so that there is no chance of it being used by accident or negligence.

This inspection sheet as well as all Kirby Morgan helmet and masks inspection and maintenance check lists can be found under notices/maintenance checklists.

February 6, 2003 URGENT SAFETY RECALL NOTICE PADI Dive Planners Recall

We've recently discovered printer errors in certain Recreational Dive Planners (RDPs).  The problems may appear in six (6) versions of the RDP, including both air and enriched air tables.  Our printer is working diligently with us to correct his error.

These tables may be included in Open Water and Enriched Air Crew Paks, book and table combinations, CD-ROM combinations and alacart tables shipped between 1 October 2002 and 31 January 2003.  Please note that not all RDPs you may have received during this period contain printer errors.

While analysis of the problems shows that the risk of injury from the errors is remote, for the sake of safety, we feel a broad approach to this issue should be taken.  We ask that you immediately check your existing inventory and remove any RDPs you received from PADI with the product numbers and production codes listed below.  The product number and production code is generally located on the bottom right hand corner on the back of the table.  The product number and production code consist of a series of numbers and letters resembling: Product

CPSC, Oceanic USA Announce Recall of Scuba Regulators

In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Oceanic USA, of San Leandro, Calif., is voluntarily recalling all Oceanic CDX first stage regulators used for scuba diving. Extreme vibration, a high oscillation of the internal components, can occur for an unknown reason within these CDX regulators, which can cause an air leakage underwater. Divers could run out of air and drown. Oceanic has received reports of noise or vibration accompanied by air leakage. No injuries have been reported.

Oceanic CDX first stage regulators subject to the recall have serial numbers ranging from 9200001 to 9205622, 0200001 to 0213294, 0D0001 to 0D3046, or 9D0001 to 9D3273. The serial number is stamped into the side of the Body. These CDX regulators were sold with various second stage regulators, including the Alpha 7, Delta 3, Gamma 2, and Zeta.

This voluntary recall does not apply to the CDX5 or any other Oceanic regulator.

Luxfer announces one-year exchange program for scuba tanks made of 6351 aluminum alloy

RIVERSIDE, Calif. (Oct. 22, 2002)-Luxfer Gas Cylinders announced a one-year exchange program for Luxfer scuba tanks made of 6351 aluminum alloy. Introduced at the 2002 Diving Equipment & Marketing Association (DEMA) trade show in Las Vegas, Nev., in October, the new Luxfer program will run throughout the 2003 calendar year.   A number of owners of older Luxfer scuba tanks made of 6351 aluminum alloy have asked Luxfer to initiate such a trade-in exchange program to facilitate the purchase of new scuba tanks made of Luxfer's proven, proprietary 6061 alloy. In response, Luxfer will implement the simple three-step program outlined below on January 1, 2003. The program will end on December 31, 2003.

The customer calls the Luxfer Customer Service Department toll-free (1-800-764-0366) and reports the number of 6351-alloy tanks to be exchanged. The Luxfer customer service representative issues a returned goods (RG) authorization number and tells the caller how to ship tank(s) to Luxfer in order to receive a special $50 credit voucher for each tank. The vouchers may be used toward the purchase of new Luxfer scuba tanks.  The customer sends 6351-alloy scuba tanks directly to Luxfer's plants in Graham, North Carolina, and Riverside, California (whichever is closest), along with the RG number and return mailing information. Luxfer mails a $50 voucher for each tank directly to the customer who returned the tanks. Each voucher may be cashed in toward the purchase of a new Luxfer scuba tank at any participating dive shop or retail store.

If you aren't sure whether your tank is made of 6351 alloy, look at the original hydrostatic test date (the earliest date) stamped on the cylinder crown. If the stamped year is 1972 through 1987, the tank is made from 6351 alloy. Some 1988 tanks were also made of 6351 alloy. If you have a 1988 tank, please call Luxfer Customer Service toll-free at 1-800-764-0366 to determine whether your tank is made of 6351 alloy.

Hazardous Tanks on the Market  -  The Department of Transportation has discovered that a substantial number of aluminum tanks have been stamped with dates in the side wall by Blue Water Divers Ltd., Tortola, British Virgin Islands.  The stamps have compromised the integrity of the walls, and the tank could explode upon filling. Some cylinders are being used in the U.S. Virgin Islands and have been sold to individuals and U.S. companies. They may have stickers covering the stamps. Remove the sticker and inspect for unauthorized markings or a marking of a month and a year such as 2 00.  Some cylinders have multiple months and years stamped on the sides. Any suspect cylinder should be condemned and not tested hydrostatically or filled.
